I/we warrant that the particulars set out herein are true and complete and I declare that I am over the age of 18years. I/we declare that I/we am not an un-discharged bankrupt or liable under any proceedings under the Insolvency Act 1967 and its amendments nor have I/we been convicted of any criminal offence involving dishonesty.

I/we understand that Kent Finance Limited are asking for personal information about me so as to use Baycorp Advantage's credit reporting service to credit check me.

I/we understand that:

• Baycorp will give you information about me/us for that purpose.
• Kent Finance Limited will give my/our personal information to Baycorp Advantage, and that Baycorp Advantage will hold that information on their system and use it to provide their credit reporting service.
• When other Baycorp Advantage customers use the Baycorp Advantage credit reporting service, Baycorp may give the information to those customers.
• You may use Baycorp Advantage's credit reporting services in the future for purposes related to the provision of credit to me/us. This may include using Baycorp Advantage's monitoring services to receive updates if any of the information held about me/us changes.
• If I/we default in my/our payment obligations to you, information about that default may be given to Baycorp Advantage, and Baycorp may give information about my/our default to other baycorp Advantage customers.

I/we authorise Kent Finance Ltd or its agent to obtain any information concerning my/our credit history from any source.
